• Must be a NY License • 2 years experience within specialty in a US facility. • Updated CPR/BCLS/ACLS/NPR/PALS (depending on your specialty) • Physical (valid for year) • PPD (if positive please provide Chest x-ray) • Titers – (copy of rebeola, rubella, mumps, varicella & hepatitis or copy of Vaccination Record) • 10 panel Drug screen • Malpractice insurance (with coverage of $1M-$6M) • Remede Consulting complete application
