Description:

Perform Diabetes Management and teaching.

Perform screening tests and records findings in patient record.

Evaluate patient nutritional management skills.

Evaluate and documents patient medication and medication management skills.

Collaborates with physicians to develop protocols for practice that define diagnostic judgments and appropriate treatment plans.

Participates in review of quality of patient care through peer record review and initiates and/or proposes necessary and/or improvements.

Identifies available community resources and refers patients accordingly.

Perform patient teaching on self-management of Diabetes complications.

Instruct Diabetes patients on insulin titration and teaching.

Evaluates and instructs patients on Glucometer usage.

Performs teaching on Diabetes lifestyle management including basic nutritional management.

Accountable for practice and professional development through continuing education and obtains appropriate number of annual CEUs necessary in maintaining certification.

Performs other duties as required.
