Need help with a COMMUNICATION Assessmnt/Care Plan

I have been given the job of coming up with an Assessment/Care Plan for Communication. The Assessment is mainly used as a tool to evaluate inpatients on a behavior unit. The "Problem" section is where I am having the biggest problem.
Alteration in Communication related to:
*memory impairment
*self isolation
*????
*????
*????
inappropriate behavior, expressive aphasia and disease process are some other "Problems" I have considered but I really think there should be more!

The "Goal" is to promote social interaction to:
*maintain organization and decision making skills
*facilitate recall of retained procedural memories
*stimulate retained language abilities & reading skills
*maintain conversation and social skills
*prevent isolation and premature deterioration of
communication skills.

The "Interventions" are easy. Attendance and Participation
in group programs of choice. There is a whole list to choose from so that they have some control over what they do.

language barroirs, culture barriors, or religious/spiritual barriors
