NC-fletcher, Responsible for Case Management of Home Health patients in collaboration with therapists, aides and SW as ordered. Requirements include RN licensure in NC. Experience in Med-Surg or other nursing for at least 1 year. Computer literacy required for point of care documentation. Demonstrated assessment and critical thinking skills. Prefer Home Health experience, and knowledge of McKesson Horizon soft
