The RN position entails making visits to homebound patients in King and Pierce counties for our growing Home Health Care Agency.
The Registered Nurse is responsible for the clinical care of assigned home care patients which may include but not be limited to case management of the patient, specific nursing tasks, patient assessment, patient / family / caregiver education and coordination with the multi-disciplinary team including RN, LPN, PT, PTA, OT, COTA, SLP, MSW and Home Health Aides.
The RN is responsible to report changes in condition to the physician or Clinical Coordinator as appropriate.
The RN reports to the Clinical Coordinator.

Nursing and Therapy team members visit patients in their homes on an intermittent basis according to doctor抯 orders.

Staff perform 5-7 patient visits daily on average.

Education and Experience Requirements:
RN / Registered Nurse
Graduate of an approved school of professional nursing with current WA license.
Minimum of one year experience as an RN
Home Health experience required
OASIS knowledge preferred
