Mt. Prospect, Position Summary : Intake Nurse is responsible for clinical evaluation of all new PAH referrals as well as telephonic clinical assessment of the patient. Intake nurse reviews referral and criteria for appropriateness of therapy. RN must have excellent communication skills to communicate with internal staff, MDs, as well as patients. Also identifies and coordinates care with home health agency nurs
