Muncie police say a local woman posed as a nurse at two Muncie area health care jobs, and last summer did the same thing at a Bluffton nursing home.
According to the probable cause affidavit filed Monday in Delaware County, Carolyn Sue Ellison, 58, Redkey, was charged with identity deception, a class D felony.
Ellison faces up to three years in prison if convicted and Muncie police say their so far three-month investigation will continue.
Investigators said Ellison impersonated a former sister-in-law, a retired registered nurse with an active license, to get a job at Meadowvale Health and Rehabilitation Center in Bluffton. She worked at Meadowvale from January until May.
Ellison, who was fired from Meadowvale for undisclosed reasons, then used her fake identification in an attempt to find employment at Parkview Nursing Center in Muncie. Someone at Parkview recognized her true identity and contacted police.
According to a report by the Indiana State Board of Nursing, Ellison, who had once been an LPN, had her license revoked in 2007 for posing as a registered nurse.
Ellison had been a fully licensed practical nurse since 1990. In January of 2007 she began the process of contacting ISBN to receive renewal forms with altered addresses and names, for a registered nurse license. Ellison eventually received and began using the fraudulent license.
In February of 2007 Ball Memorial Hospital administrators discovered a discrepancy with the falsified documents Ellison presented. Specifically, the name and address she provided for her license did not match records already on file. She had been employed there as a practical nurse since 2005, but had been signing documents as a registered nurse. She was terminated from the hospital in February 2007.
Before her licensing issues, Ellison worked at Parker Health Care and Rehabilitation as well as Ball Memorial Hospital.
Ellison could face similar charges in Bluffton for falsifying her identity, though so far Muncie police have not contacted Bluffton investigators.
Ellison could also face federal Social Security fraud charges because, police said, she was receiving disability payments while she was employed.
Due to privacy laws regarding former employees, Meadowvale Health and Rehabilitation Center was only able to release a statement confirming Ellison no longer works at the facility and that they have and will continue to cooperate with law enforcement in this matter.
