Need help w/ a Communication Assmnt/Care Plan

I am having difficulty with the "Problem" section. I have come up with: Alteration in Communication related to:
*expressive aphasia
*self isolation
*memory impairment
*????
*????
What else can I put here?? The Assessment/Care Plan is "generic" for a group of inpatient behavior patients that range from bipolar to MRDD to anoxic encephalopothy.

The section of the care plan for "Goals" and "Interventions" is complete. I really need some help with the "Problem" section.
