Hi everyone, i am glad to be here


Hi everyone
i am glad to be here ,improve the knowledge of dialysis, i work in NKF in Singapore, also i am new learner about dialysis, just 9 months, our work is very busy, one nurse ratio 4-5 patients,anyway the patients are quite stable, i would like appraciate if somebody would give me some advice how to summary the monthly report about patient general condition on dialysis?

We use a check off list.

Complient with treatments Yes ___ No ___ # of missed tx ___

Able to achieve EDW Yes ____ No ____

Access Fistula ___ Graft ___ Catheter ___

Trouble with access Yes____ NO ____

Hospitalization Yes ____ No ____ Reason ___________________

General condition for the month Stable, Recovering etc.


Our computer has a printout of any time frame you desire with all the info needed to fill out the above from.

i.e 02/01/2207 - 02/28/2007. It has all the pre and post info. B/P's pre/post wts, wt gain, EDW at the top, etc.

I am at a Fresenius unit now but Davita has a similiar, although much more extensive report, with labs etc on it too.

We put it sideways on the page and can get 3-4months on one sheet of paper so we can see any trends.
