paperwork

dont you get fed up with it all. buzz words legal document / if it isnt written down it didnt happen etc nursing care deteriorating because emphasis changed by managers +++++ trying to justify thier existance

Actually, it's more hospitals trying to justify their charges. For example, if an IV infusion on a pump isn't mentioned for 24 hours, the insurance company won't pay for 24 hours of IV therapy.

Of course, there are a gazillion other reasons - legal, financial, etc., etc.

Don't forget that good charting protects you in a lawsuit. As far as the legal system is concerned if it isn't writen it isn't done. I've been depositioned in a case where the original incident occured 4 years prior. - accurate charting saved my arse!!!!!!! good documentation is part of good patient care - It's a pain and time consuming especially in long term care a psych where 1 RN typically takes care of 15 to 25 patients. ARRRRGGGGGGGG!

my chart is good for 21 years because if somebody shoots you when he's 17 it must have been because of a birth trauma. I'm waiting for somebody to start suing OB's because their kid has ADD.

People do sue for things like their kids having ADD. ADHD, Poor score on college entrance exams -"Must have been that fetal anoxia!" That's why fewer and fewer OB's are doing deliveries because they have the highest malpractice insurance in medicine - also why most docs reccomend C-sections! Sad really
