care planning advice

I apologise in advance as I feel really dumb asking (as I am a second year student) but I need some advice as I am a little lost. I have to do three careplans for patients on my placement. However I have never done a care plan before and community nursing is not the same as hospital so I am unsure where to start. I know all about assess, plan implement and evaluate and the ADL's and goal setting etc. But in the community they use single assessment plans and in the nursing assessment the ADL's are not included. Any advice on how to go about starting one would be really appreciated.
