Pass Nclex-question Of The Day

A client comes in to the emergency room complaining of persistent cough, hemoptysis, hoarseness, weight loss and fatigue. Which of the following actions, if taken by the nurse, is MOST appropriate?

a. Auscultate the lungs
b. Place client in an isolation room
c. Take vital signs
d. Administer oxygen therapy.

Correct Answer is B

Rational

The client is exhibiting sign and symptoms of tuberculosis. It is MOST important to implement Airborne Precautions and place client in respiratory isolation.

d. No indication for oxygen at this time

that makes me more terrible, i think, now that its already been a critical situation to me:what if i dont pass the exam on time?

but anyway, this question is helpful, i'll note this down.hihihihihi
