Patient teaching

Do you do any patient teaching about medications, even ones the patient may have been on for some time?

We make it a practice to review all medications with patients including ones that have been in use for some time. Very often there are things that have been forgotten like why it's best to take time released channel blockers at night, being aware of vitamin K rich foods when taking warfarin especially when fruits and veggies are in season or more importantly, when there are synergistic effects that occur with new medications like fluroquinolones and antiarrythmics

I always try to take advantage of every teachable moment. In my experience, and from what I heard, many nurses are doing drug teaching, however they just don't document it.
