Adventures in charting

Tonight was rough at work, but there was one diamond. While looking up meds/Dxs for a new admit, another nurse and I noticed that one of her admitting DXs was: Multifart dementia.
We fell all over laughing, we couldn't even speak to tell our supervisor why we found her admit paperwork so funny.

The next great bit is this - while doing my clinical rotation at a local hospital (I'm an LPN/Student RN) I was reviewing a psych pt's H/P and noticed that his doctor had written "Pt has commited suicide three times". I chuckled so loud that I attracted the attention of a nearby resident. I pointed out that this was one resilliant pt, and she and I have shared information and occasional insights like this ever since.

One of my favs was a fellow LPN who I worked with after school...We worked nights and she used to chart

"Patient had a quite night but was confussed."

Those were her exact words..
